• The complainant's name and personal data should be included in the complaint; if the complaint is submitted by proxy, the latter's name and personal details should be added.
• The complainant must indicate the subject and basis of his complaint unless inclusive of specific facts worthy of inquiry and inspection.
• The complaint must be signed by the applicant or his attorney.
• If the complaint is submitted by an attorney, the power of attorney (original and copy) must be submitted together with the attorney’s ID.
